29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about 04/10/13 at Eatonton, GA: Employees were exposed to fall hazards while performing framing work on a new residential home.  The employer failed to provide adequate fall protection in the following instances:       a. Truss Installation:  An employee was exposed to fall hazards when framing a truss, and standing on a wooden plank at a height of 12 feet above the first floor level without fall protection.       b. Floor Opening, Front Entrance:  Employees were exposed to fall hazards while performing framing work in the vicinity of the front entrance to the house.  The employer failed to install a floor cover to protect employees from falls of 10 feet from ground level to the basement below.          c. Floor Opening - Basement Stairway: Employees were exposed to fall hazards while performing framing work in the vicinity of an unguarded floor opening.  The floor opening was constructed to install a stairway that connected the first floor to the basement.  The employer failed to install guardrails around the floor opening to prevent falls of 9 feet to the basement below.       d. Window Openings:  Employees were exposed to fall hazards, while performing framing work in the vicinity of four unprotected window openings located on the first floor of the house. The employer failed to install top rails in first floor window openings to prevent falls of 10 feet to the ground below.       e. Wall Openings:  Employees were exposed to fall hazards while performing framing work in the vicinity of two door openings which were located on the first floor of the house.  The employer failed to install guardrails systems on the two doorway openings that lead to the rear of the house, at a height of 10 feet to the ground below.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about 04/10/13 at Eatonton, GA: Employees were exposed to fall hazards while performing framing work at a residential home.  The employer failed to provide fall protection training certification records for each employee working at heights above 6 feet from the ground.
