29 CFR 1910.119(f)(4): The employer did not implement safe work practices for employees performing high risk maintenance activities on process equipment. a) On April 17, 2013 or at times prior thereto in the Catalytic Hydro-Desulfurization Unit 2 (CHD-2) at the ExxonMobil Beaumont Refinery, the employer did not ensure that the E-1 exchangers were free of all energy sources before employees began flame cutting to remove flange bolts from the channel heads. b) On April 17, 2013 or at times prior thereto in the Catalytic Hydro-Desulfurization Unit 2 (CHD-2) at the ExxonMobil Beaumont Refinery, the employer did not implement safe work practices for safely opening process equipment, such as but not limited to the E-1 Exchangers.

On April 17, 2013 or at times prior thereto in the Catalytic Hydro Desulfurization Unit 2 (CHD-2) at the ExxonMobil Beaumont Refinery the employer did not assure that each of the employees followed the safety rules of the facility including safe work practice for removing flange bolts in order to remove the channel heads of the E-1 heat exchangers that contained petroleum hydrocarbons. a) The employer failed to assure that the E-1 exchangers were free of all energy sources before employees began flame cutting to remove flange bolts from the channel heads. b) The employer failed to assure that employees implemented Maintenance Procedure 203 (MP-203) to safely open process equipment.
