29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On 4/22/2013, employees working at Jacobson Companies located at 18429 Quarry Road, Warrenton, MO 63383 were using a horizontal band saw, serial # 911121, in the maintenance area, which had about 10 inches of the bade exposed. Additionally, employees were using a Sears drill press, no serial number available, in the maintenance area and had the drill bit exposed without a guard.

29 CFR 1910.23(e)(1): A standard railing did not consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The intermediate rail was not approximately halfway between the top rail and the floor, platform, runway, or ramp. On 4/22/2013, employees working at Jacobson Companies located at 18429 Quarry Road, Warrenton, MO 63383, were exposed to a fall hazard. The landing for the maintenance employee break area was missing an intermediate rail halfway between the top rail and the flooring/platform.

On 4/22/2013, employees working at Jacobson Companies located at 18429 Quarry Road, Warrenton, MO 63383, were exposed to a fall hazard. The stairway for the maintenance employee break area was missing an intermediate rail halfway between the top rail and the stairs.
