29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i) - (ii):   Jobsite located at 13612 Cumming Street: The employer is failing to protect employees from potential fall hazards associated with working from a scaffold platform less than 10 feet to the ground below. Employees were using a tubular framed scaffold which was not fully decked or planked at all working levels to perform an EFIS installation; exposing the employees to a fall.   Jorge Mireles - Lazalde was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1)(ii), which was contained in OSHA inspection number 311469035, citation number 1, item number 1 and was affirmed as a final order on 12-1-2009 , with respect to a workplace located at 19363 Woolworth Ave. Omaha, NE 68130.  Jorge Mireles - Lazalde was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1)(ii), which was contained in OSHA inspection number 314055617, citation number 2, item number 1 and was affirmed as a final order on 08/13/2010 , with respect to a workplace located at 18710 Honeysuckle Drive, Omaha, NE 68022.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  Job site located at 13612 Cumming Street, Omaha, NE: The employer is failing to protect employees from potential fall hazards associated with working from a scaffold platform approximately 6 feet from the ground below. Employees were conducting EFIS and masonry work off of tubular framed scaffolds which were missing base plates on the leg posts.  Jorge Mireles - Lazalde was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(c)(2), which was contained in OSHA inspection number 311469035, citation number 1, item number 2a and was affirmed as a final order on 12-1-2009 , with respect to a workplace located at 19363 Woolworth Ave. Omaha, NE 68130.  Jorge Mireles - Lazalde was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(c)(2), which was contained in OSHA inspection number 314055617, citation number 2, item number 2 and was affirmed as a final order on 08-13-2010, with respect to a workplace located at 18710 Honeysuckle Drive, Omaha, NE 68022.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   Job site located at 13612 Cumming Street, Omaha, NE: The employer is failing to protect employees from potential fall hazards associated with working from a scaffold platform approximately 6 feet from the ground below. The scaffolds being used had not been inspected prior to utilization by a person trained to meet the requirements of competent person as set forth by the standard. Employees were conduction EFIS and masonry work off of tubular frame scaffolds which had numerous issues including, but not limited to:      a) Missing base plates      b) Working levels not fully planked   A proper inspection by a competent person would have revealed these issues allowing the workers to correct them prior to working on the scaffolds.   Jorge Mireles - Lazalde was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(f)(3), which was contained in OSHA inspection number 311469035, citation number 1, item number 2b and was affirmed as a final order on 12-1-2009 , with respect to a workplace located at 19363 Woolworth Ave. Omaha, NE 68130.

Job site located at 13612 Cumming Street, Omaha, NE: The employer is failing to ensure employees who are working and erecting scaffold platforms are trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffolds being used and to understand the procedures to control or minimize those hazards. Employees were conducting EFIS and masonry work off of tubular framed scaffolds which had numerous violations of the scaffolding standard.
