29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he began work, and did not remove those devices when he stopped working on the machine or equipment being serviced or maintained:  a. Instances existed where more than one employee performing lockout work on the same equipment, at the same time, did not have his personal lock on the energy control device.  b. At least one instance existed where an employee allowed another employee's lock to remain on the energy control device when performing lockout work, did not place his own lock for energy control.  Abatement documentation is required for this violation.
