29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by this paragraph (l): a. On or about 4/16/2013, Glazers Premier Dist. LLC - warehouse area, 57-C Eliza's Retreat, St Croix, VI: Power industrial truck were used on a daily basis to transport warehouse materials during loading/unloading operations. The employer did not certify that each operator had been trained and evaluated. Note 1: The Glazers Premier Dist. LLC , DBA Premier Wines & Spirits LTD was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(6) which was contained in OSHA inspection number 313533952, citation number 1, item number 1, and was affirmed as a final order on or about 5/8/2011, with respect to a workplace located at 7020 Estate Bovoni, St. Thomas U.S.V.I. Note 2: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
