29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: JOBSITE: 1601 Air Park Drive, Farmington, MO 63640, at this location in the mixing area a portable fan, Big Country Breeze, BCB-20F-DDF, was being used with a defective electrical cord. A visual inspection revealed a missing ground pin. The Molding Company dba Forte Product Solutions was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334(a)(2)(ii), which was contained in OSHA inspection number 313637050, citation number 3, item number 1, issued on September 2, 2009 and was affirmed as a final order on September 28, 2009, with respect to a workplace located at 1601 Air Park Drive, Farmington, MO 63640.
