29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Jobsite, Homesite 2B, Laurelwood Plan, Vanadium Road, Pittsburgh, PA:  On or about April 24, 2013, an employee was working on a residential roof without fall protection.  The employee was sheathing the roof and was exposed to a fall hazard of approximately 13 feet to the ground below.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee:  a) Jobsite, Homesite 2B, Laurelwood Plan, Vanadium Road, Pittsburgh, PA:  On or about April 24, 2013, an employee was working on a residential roof above 6 feet in height and was not trained in fall protection.  The employee was sheathing the roof and was approximately 13 feet above the ground.
