29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.     WORKSHOP:       1. Combustible wood dust was allowed to accumulate on the floor in and around the RUWO Stringer machine, air hose, back of the machines, electrical wires, beams, and work table. The dust layers were allowed to accumulate to sufficient depth and spread over a sufficient surface area to create a fire/explosion hazard.       2. Combustible wood dust was allowed to accumulate on the floor in and around the Rockwell Radial Arm saw, saw, work table, beams, electric motor. The dust layers were allowed to accumulate to a sufficient depth and spread over a sufficient surface area to create a fire/explosion hazard.     3.  Combustible wood dust was allowed to accumulate on the tops of the walls, beams, floor joists, light fixtures, and piping.  The dust layers were allowed to accumulate to sufficient depth and spread over a sufficient surface area to create a fire/explosion hazard.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.     WORKSHOP:  Non-classified electrical equipment, such as (but not limited to) non-gasket control boxes, extension cords, electric motors and switches rated at 120 V/60 HZ alternating current were installed and used throughout the work area where combustible and explosive wood dust was not only generated, but also accumulated.      The extension cords attached to the Delta 50-760 dust collectors for the woodworking machinery were not approved for nor safe for use in a Class II, Division 2 hazardous location.
