29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): 1 Main St, Whitinsville, MA On 4/24/13 and at times prior, employees manufacturing wood, acrylic, and fiberglass products were exposed to amputation hazards while operating the following machines with inadequate point of operation guarding: Instance 1. Heian CNC Router. Employees were exposed when guards were removed for cutting polished metal products. Employer abated by forbidding guard removal for any operation. Instance 2. Grizzly Table Saw. Employees were exposed when operating machine with guard and feed rollers removed. Employer abated by adding a guard and training employees to position the guard when feed rollers were not used. Instance 3. SC MiniMax S3 Table Saw. Employees were exposed while using without guarding. Employer abated by installing a fixed guard. Instance 4. Powermatic Band Saw. Employees were exposed to a portion of unguarded blade above the bottom of the guide rollers. Employer abated by adding a guard extending to the bottom of rollers. Instance 5. SCMI R8 Router Machine. Employees were exposed due to no point of operation guarding. Employer abated by adding a fabricated polycarbonate guard. Instance 6. Powermatic Drill Press (green rounded shape.) Employees had access to the unguarded point of operation. Instance 7. Powermatic Drill Press (green squared shape.) Employees had access to the unguarded point of operation. Employer abated by adding a telescoping guard. Instance 8. Delta Drill Press. Employees had access to unguarded point of operation. Employer abated by permanently removing machine from service. Instance 9. Powermatic Table Saw. Employees were exposed while operating without the guard installed.
