29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used: (a) 755 West Boylston St Worcester, MA: On or about April 24, 2013, employees were observed using a scissor lift type scaffold to access the roof work area, which was 5 FT above the scaffold platform, and therefore not in accordance with this subpart.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was notprotected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) 755 West Boylston St Worcester, MA: Employees were observed working to remove the corrugated steel decking surface making up the roof of this structure more than 19FT above the ground below without any fall protection, which is not in accordance with this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
