29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following: a) Xerox Commercial Solutions, LLC, 244 Dundee Ave, Greeley, CO 80634: On and before April 24, 2013 the employer failed to develop a written hazard communication program which at least describes a list of chemicals, material safety data sheets, and employee information and training. The employer utilizes the following chemicals for regular cleaning use and vermin control. This condition exposed employees to chemical health hazards. Regular Use: (1) Cleaning agents (2) Paints (3) Air fresheners/Epoxy's (4) Degreasers (5) Primers (6) Bug Killers (7) Floor finish/stripper Occasional Use: (1) Temprod SC Insecticide; (2) Steri Fab; (containing Isopropanol) (3) Deltadust Insecticide

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training was not designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets. a) Xerox Commercial Solutions, LLC, 244 Dundee Ave, Greeley, CO 80634: On and before April 24, 2013, the employer did not provide effective information and training to employees on the physical and health hazards associated with chemicals used regularly and vermin control chemicals. Employees were unaware of the health hazards associated with chemicals, were unable to identify how the bed bugs were treated, and were unable to locate material safety data sheets. This condition potentially exposes employees to chemical hazards. The following chemicals were used: Regular Use: (1) Cleaning agents (2) Paints (3) Air fresheners/Epoxy's (4) Degreasers (5) Primers (6) Bug Killers (7) Floor finish/stripper Occasional Use: (1) Temprod SC Insecticide; (2) Steri Fab; (containing Isopropanol) (3) Deltadust Insecticide
