29 CFR 1926.404(b)(1)(ii):  Where an assured equipment grounding program was not utilized, receptacles were not protected with ground fault circuit interrupters when on a two-wire, single- phase portable or vehicle-mounted generator rated more than 5kW, or where the circuit conductors of the generator were not insulated from the generator frame and all other grounded surfaces:    a)  Employees were using a Honda EG 2500 series 120/240 volt single phase three-wire gas powered portable generator with receptacles not protected with ground fault circuit interrupters to power tools onsite.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:      a)  Employees were not wearing protective helmets where there was a possible danger of head injury from falling objects.      The Site Maintenance, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 311253652, citation number 1, item number 1 and was affirmed as a final order on December 3, 2008, with respect to a workplace located at 656-660 Lancaster Circle, Elgin, IL.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      a)  Employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 17.6 feet above the ground.      The Site Maintenance, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 312624620, citation number 1, item number 2 and was affirmed as a final order on November 12, 2008, with respect to a workplace located at 1418 Seven Pines Rd. Schaumburg, IL.    The Site Maintenance, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 311253652, citation number 1, item number 2 and was affirmed as a final order on December 3, 2008, with respect to a workplace located at 656-660 Lancaster Circle, Elgin, IL.
