29 CFR 1926.501(b)(1):   Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about April 22, 2013 at the above addressed site, employees performing work removing existing glass and aluminum framing on a walking/working surface with unprotected edges approximately 7 ft. 9 inches above a lower level were not protected from falling to that lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 22, 2013 at the above addressed site, employees using an aluminum extension ladder to gain access to and from a walking/working surface approximately 7 ft. 9 inches above a lower level for the removal of existing glass and aluminum framing did not have the side rails of that ladder extended at least 3 ft. above the landing surface.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  On or about April 22, 2013 at the above addressed site, Employees gaining access to and from a walking/working surface for the removal of existing glass and aluminum framing approximately 7 ft. 9 inches above a lower level using an aluminum extension ladder set up on an active sidewalk did not have that ladder secured to or barricaded to pevent accidental displacement.
