29 CFR 1926.651(g)(1)(i): Where oxygen deficiency (atmosphere containing less than 19.5 percent oxygen) or a hazardous atmosphere existed or could reasonably be expected to exist, such as in excavations in landfill areas or excavations in areas where hazardous substances are stored nearby, the atmospheres in the excavation shall be tested before employees enter excavations greater than 4 feet:   a) Area of 1502 Second Street, Bethlehem PA - On or about March 11, 2013, employees working in an excavation to repair a high capacity sewage line, at a depth in excess of six feet below grade with a potentially hazardous atmosphere and/or an oxygen deficiency, did not conduct atmospheric testing of the excavation prior to the start of work.
