29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     On or about April 23, 2013, at the above address jobsite, an employee using a Hitachi 1 ¾ pneumatic coil nailer to secure felt paper was not wearing eye protection equipment while exposed to a potential struck by eye injury.

29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, shall be secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder:     On or about April 23, 2013, at the above address jobsite, employees using a portable ladder set up on a construction site outside an active doorway to gain access to and from a steep pitch roof approximately 8 feet 9 inches above a lower level did not have that ladder secured or barricaded to prevent accidental displacement.
