29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   On or about April 22, 2013, at 1502 Sam Nunn Blvd, Perry, Georgia:   Two employees performing framing work were exposed to a fall hazard of approximately 44-feet and greater when a Lull 1044-C-54-Series 2, powered industrial truck was used to lift them in a job made work platform that was not approved by the manufacturer.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 22, 2013, at 1502 Sam Nunn Blvd, Perry, Georgia:  a) Three employees performing framing work were exposed to a fall hazard of approximately 44-feet and greater when they worked at the unprotected edge of the building without using fall protection.   b) Two employees were exposed to a fall hazard of approximately 34-feet when they worked near an elevator shaft that did not have a guard rail installed.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about April 22, 2013, at 1502 Sam Nunn Blvd, Perry, Georgia:  Twelve employees performing framing work were exposed to fall hazards of up to 44-feet and greater when the employer did not train the employees how to recognize and minimize fall hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about April 22, 2013, at 1502 Sam Nunn Blvd, Perry, Georgia:  Six employees performing framing work were exposed to a fall hazard of approximately 9-feet when they used one-half of an aluminum extension ladder that was not set three-feet above the roof landing.
