29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by this paragraph (l): (a) Warehouse, shipping and receiving department. On or about Aprli 25, 2013 an employee operating Raymond forklift truck was not certified for training and evaluation of his performance and skills for safe operation that had to indicate operator's name, the date of training, the date of evaluation, and the identity of the person(s) performing the training and evaluation. There is no abatement certification or documentation required for this item.
