29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:   a)      Employees were exposed to work activities that emitted noise levels that exceed the 8-hour time-weighted average (TWA) of 85 dBA.    All provisions of 29 CFR 1910.95 (c) through (n) must be covered in a hearing conservation program. Key elements include, but are not limited to, the following:  1)      Noise level monitoring 2)      Audiometric testing 3)      Wearing of hearing protection 4)      Training Program 5)      Providing copies of standard to employees  6)     Recordkeeping    ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:   a)      Employees were exposed to work activities that emitted sound levels that exceed the 8-hour time-weighted average (TWA) of 85 dBA and had not been given audiograms.    ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.1026(d)(2)(iii): The employer used the scheduled monitoring option, and monitoring revealed employee exposures to chromium (VI) to be at or above the action level; however, the employer did not perform periodic monitoring at least every six months:  a)      On 4/25/2013, employees were exposed to hexavalent chromium created during the electroplating operation of Chrome.  Initial sampling conducted in 2012 indicated results over the action level for hexavalent chromium. OSHA conducted sampling during the inspection and results indicated over exposure greater then the action level of 2.5 micrograms/m3.

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such activities. Employer did not require employees to wear eye protection that complies with:       - ANSI Z9.1-1951 (revised 1971) American National Standard Practice Z9.1 Section 14.3 states that whenever there is danger of splashing engaged employees shall be required to wear tight-fitting chemical goggles or an effective face shield.     a)      On 4/25/2013, employees were exposed to splash hazards when chemical were transferred to and from the tanks and when additions were made to the tanks.     ABATEMENT CERTIFICATION REQUIRED.
