29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Northwest Corner Window - On or about April 10, 2013 an employee was exposed to a fall hazard while working near a window 6 feet 6 inch above the ground. Employee was not protected from falling by the use of guardrail systems, safety nets systems or personal fall arrest systems (b) West Stairwell Access Door - On or about April 10, 2013 an employee was exposed to a fall hazard while working near an unguarded doorway 6 feet 6 inch above the ground. Employee was not protected from falling by the use of guardrail systems, safety nets systems or personal fall arrest systems.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, shall be equipped with: 29 CFR 1926.1052(c)(1)(i): At least one handrail; and 29 CFR 1926.1052(c)(1)(ii): One stairrail system along each unprotected side or edge. Note: When the top edge of a stairrail system also serves as a handrail, paragraph (c)(7) of this section applies. (a) West Stairwell - On or about April 10, 2013 an employee was exposed to a fall hazard while walking up and down a flight of stairs with no stairrail system installed. Employee was exposed to falling 23 feet from third floor to lower level.
