29 CFR 1926.451(b)(5)(ii):  The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:    (a) Left Side of the Building - on or about April 25, 2013 - the plank on the right side ladder jack scaffold extended approximately 24 inches past both support brackets.

29 CFR 1926.451(g)(1)(i):  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a) Left Side of the Building - on or about April 25, 2013 - two ladder jack scaffold systems were not equipped with personal fall arrest systems for the employees that were performing roofing work from these systems.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    (a) Left Side of the Building - on or about April 25, 2013 - the right side ladder jack scaffold system was not equipped with an egress ladder.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) Building Roof - on or about April 25, 2013 - no conventional fall protection was available for the employees that were performing roofing work.  Ground to roof deck elevation was 22 feet.  Roof pitch was 12 on 12.
