29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:     On or about April 25, 2013, employees were exposed to being struck by welded scaffold frames up to approximately 18 feet in height, when not wearing hard hats, while moving the two sections in width, assembled scaffold.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     On or about April 25, 2013, employees were exposed to falls of up to approximately 14 feet in height to the ground level below, when working from the work platform of the scaffold, which had only two scaffold planks and approximately 3 feet apart on the second level, while installing the brick veneer to the front of the residential home.

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end:     On or about April 25, 2013, employees were exposed to falls of approximately 14 feet in height to the ground level below, when employees stand on the cantilevered end of the work platform that extended approximately 24 inches beyond the scaffold frame support, while installing the brick veneer to the residential home.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:     On or about April 25, 2013, employees were exposed to falls of approximately 14 feet in height to the ground level below, when working from the scaffold which did not have a ladder to the upper levels, while accessing and egressing the scaffold platforms.
