29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about 04/25/2013 - Forming Department, a Lockout/Tagout (LOTO) procedure was not developed, documented, and used by employees clearing a blocked Pug Mill extruder during startup operations. The blockage or material jam clearing procedure used did not meet the exception from LOTO requirements allowed at 29 CFR 1910.147(a)(ii)(B) because effective alternative safety measures were not used and the employee, i.e. the operator was allowed to place his body in "an associated danger zone" that existed during this maintenance operation.
