The screwgun was connected to 120-volt ac power via a series of extension cords, the first of which was plugged into a wall receptacle in the basement. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.    a) On or about 04/25/13, on commercial roofing construction site, 3748-3750 Harlem Rd., Cheektowaga, NY: Employees used a DeWalt Model DW235G Screwgun to install wooden boards on the parapet walls. The screwgun was connected to 120-volt ac power via a series of extension cords. The second last extension cord in the series ran under the outside door to the basement. The door was closed onto the cord pinching the outer sheath of the cord.   NO ABATEMENT CERTIFICATION REQUIRED
