Dantas Construction, LLC @ 1015 Wildwood Rd., Avon, CO 81620: On and before 4/24/13, the controlling employer did not maintain a safety program that allowed for frequent and regular worksite inspections with the intent of identifying workplace hazards. A sub-contracted employee was exposed to fall hazards while working on the south fabricated frame scaffold system with incomplete guardrail system. This condition exposed the employee to fall hazard of approximately 12 ft.   (b) D.W. Dantas Construction, LLC @ 1015 Wildwood Rd., Avon, CO 81620: On and before 4/24/13, the controlling employer did not maintain a safety program that allowed for frequent and regular worksite inspections with the intent of identifying workplace hazards.
