Phillips Construction, 280 South Main Street, Cambridge Springs, PA  16403, on or about April 23, 2013 and times prior thereto:  An employee was performing carpentry tasks with a powered circular saw.  The employee was wearing standard sun glasses that were not ANSI Z87 approved.

29 CFR 1926.21(b)(2):   The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  A.)  Phillips Construction, 280 South Main Street, Cambridge Springs, PA  16403, on or about April 23, 2013 and times prior thereto:  Employees were not provided with electrical safety training on items that included, but were not limited to, use of GFCIs, identification of electrical cord / equipment damage, and use of electrical extension cords.  The employees were utilizing electical power tool equipment and electrical extension cords.  B.)  Phillips Construction, 280 South Main Street, Cambridge Springs, PA  16403, on or about April 23, 2013 and times prior thereto:  Employees were not provided with personal protective equipment (PPE) training covering topics including, but not limited to, when PPE is necessary, what types of PPE are necessary, how to properly adjust and wear PPE, limitations of PPE, care and maintenance of PPE, and the useful life and disposal of PPE.

29 CFR 1926.404(b)(1)(i):   Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  A.)  Phillips Construction, 280 South Main Street, Cambridge Springs, PA  16403, roof of the structure, on or about April 23, 2013 and times prior thereto:  Employees were utilizing electrical powered equipment, including, but not limited to, a hammer drill to drill holes in concrete to accept molly bolts.  The electrical power for the hammer drill was provided via electrical extension cords.  The hammer drill and the extension cords were not protected by GFCI and/or an AEGP.  B.)  Phillips Construction, 280 South Main Street, Cambridge Springs, PA  16403, roof of the structure, on or about April 23, 2013 and times prior thereto:  Employees were utilizing electrical powered equipment, including, but not limited to, a power saw to cut 2' x 4' lumber.  The electrical power for the power saw was provided via electrical extension cords.  The power saw and the extension cords were not protected by GFCI and/or an AEGP.

29 CFR 1926.501(b)(10):   Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  A.)  Phillips Construction, 280 South Main Street, Cambridge Springs, PA  16403, roof of the structure, on or about April 23, 2013 and times prior thereto:  Employees were performing work activities on the flat roof of the structure without a means or method of fall protection when exposed to a fall hazard of 12.7 feet to the ground below.  Work activities included, but were not limited to, application of Truco Eterna-Seal seam sealer to the membrane roof.
