29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  WORKSITE: The scrap lumber and roofing debris with protruding nails was not kept clear in and around the bottom of the materials hoist that was being used in lieu of a ladder by employees to access the roof.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  WORKSITE: The employees installing plywood sheathing on the roof using pneumatic nailing devices were not wearing eye protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   WORKSITE: The employees performing roofing activities on the 7/12 pitch roof were not protected from the hazard of falling between 17 feet, 10 inches and 19 feet, 7 inches to the ground by the use of appropriate fall protection devices/systems.

29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows as part of a complete personal fall arrest system which maintains a safety factor of at least two:  WORKSITE: A peak bracket anchorage point installed on the roof, had two lifellines installed on the anchorage and was used as part of a personal fall arrest system for two employees on the roof.

29 CFR 1926.552(a)(1): The employer shall comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators. Where manufacturer's specifications are not available, the limitations assigned to the equipment shall be based on the determinations of a professional engineer competent in the field.  WORKSITE: The material hoist, no name or serial numbers was used by employees in lieu of a ladder to access the roof.
