29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.   On or about April 26, 2013, employees who were performing stucco work were exposed to a fall hazard.  Wooden scaffold planks were not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide (fully decked).  The fall distance to the ground where employees worked at different locations on the scaffold was approximately 15 feet.

29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired.  A.    On or about April 26, 2013, an employee performed stucco work from a scaffold that had been installed with cross-brace rods that were bent.  This condition exposed employees to fall and crushing type hazards should the scaffold collapse.  This scaffold was located near the northeast end of the building.  The fall distance from the working level where the employee worked to the ground below was approximately 15 feet.  B.    On or about April 26, 2013, an employee performed stucco work from a scaffold that had been installed with cross-brace rods that were bent.  This condition exposed employees to fall and crushing type hazards should the scaffold collapse.  This scaffold was located just west of the scaffold near the northeast end of the building.  The fall distance from the working level where the employee worked to the ground below was approximately 15 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.   On or about April 26, 2013, an employee performed stucco work without a means of fall protection.  The fall distance from the working level where the employee worked to the ground below was approximately 15 feet.
