29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:   Building 340: On April 25, 2013, and at times prior thereto, employees were exposed to alkali skin burns while handling and mixing Portland cement without gloves, with cotton gloves, and with short sleeve shirts.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:   Building 340: On April 25, 2013, and at times prior thereto, employees were exposed to fall hazards of approximately 6.5 feet up to approximately 25 feet and subsequently becoming struck by collapsing scaffolding which was not fully cross braced.
