29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: (a) Trench, new storm water management facility located between Lots #5 & #6 on South Sherwood Road, Smyrna, GA 30082: Employees were allowed to install concrete storm sewer lines in a trench that was 5-feet, 10-inches wide by 6-feet to 6-feet, 9-inches deep by 40-feet long without an inspection of the excavation by a competent person. Employees were not provided cave-in protection, a means of egress, or head protection prior to the start of work within the excavation.
