Storage Area - On or about April 26, 2013, portable metal ladders were utilized to do inventory counts were not maintained in good usable conditions. The conditions of the portable metal ladders were as follows exposing employees to a fall hazard;   1. The 5-step metal ladder had two (2) tubular metal base frame rods bent. In addition, two (2) of the ladders' rubber pads were damaged.    2.  The 3-step metal ladder showed its left metal frame side bent.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.   a. Press Brake Area - On or about April 26, 2013, employees were required to operate a - Pirahana Ironworker (Serial # SEP 65/140) - foot activated press brake which was inadequately guarded. Employees were thereby exposed to cuts, lacerations, and amputations.
