29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employees exposure to bloodborne pathogens:    (a) Brookstone Ambulatory Surgery Center- The syringes, scalpels, and some of the IVs used at the facility were not equipped with engineered sharps protection exposing employees to the increased risk of exposure to bloodborne pathogens due to contaminated needle sticks and other percutaneous injuries.

29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually:  (a) Brookstone Ambulatory Surgery Center- The employer did not ensure that the annual review of the bloodborne pathogen exposure control plan was documented.  The program did not include a review date.   (b) Brookstone Ambulatory Surgery Center - The employer did not ensure that the annual review of the bloodborne pathogen exposure control plan documented the consideration and implementation of safe sharp devices.
