A) East side of building, on or about April 25, 2013, Window openings on the second floor were not properly guarded, exposing employees to a fall hazard of approximately 12 feet.

A) Job site, on or about April 25, 2013, employees were using a Case forklift and a Genie rough terrain forklift that did not have a functioning horn, exposing employees to struck-by hazards. B) Job site, on or about May 9, 2013, employees were using a forklift that had a damaged side mounted rearview mirror, exposing employees to struck-by hazards.
