The program did not include a current revision date. The plan was dated October 2009 and still had the previous owners name at the bottom.  (b)  Genesis Healthcare Cartersville Heights Center- The employer did not ensure that the annual review of the bloodborne pathogen exposure control plan documented the implementation of safe sharp devices by listing the devices in use at the facility.   Safe sharp devices were listed but the list was not complete. The retractable lancets and the Magellan insulin safety syringes in use at the facility were not listed.
