29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour. Worksite: Compressed oxygen cylinders were not separated a minimum distance of 20 feet from acetylene cylinders.

29 CFR 1926.850(c): All electric, gas, water, steam, sewer, and other service lines were not shut off, capped, and/or otherwise controlled, outside the building line before demolition work was started. Worksite: The employer did not ensure that all high powered electrical transmission lines located within close proximity of the building being demolished were removed or relocated prior to the start of work. On or about 4/20/2013 a power transmission line was struck and severed by debris from a building being demolished.
