29 CFR 1926.403(e): Splices. Conductors shall be spliced or joined with splicing devices designed for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices shall first be so spliced or joined as to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors shall be covered with an insulation equivalent to that of the conductors or with an insulating device designed for the purpose.   a.  Located at Progressive Bank, there was an employee operating a Makita 5007F, serial number 0929179, portable circular saw in order to cut roofing material that had a flexible cord that was spliced by intertwining the cord's conductors and using electrical tape as the cords insulation, which did not provide the employee any protection from an electrical shock hazard.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a.  Located at Progressive Bank, there were employees preparing materials for employees doing roofing work near a section of the bank's basement that extended from the building which provided no means to protect the employees from a fall, thereby exposing the employees to a fall hazard at a maximum height of 10 feet into the basement.
