29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Facility - on or about 4/30/13, employees perform maintenance and servicing, such as replacement of saw blades, on a Marvel Saw, which have electrical/480 V and hydraulic energies.  The employer had not developed an equipment-specific procedure for the sas, which need to have both energy sources (electrical and hydraulic) reduced to a zero-energy state and locked out, prior to the saw blade change out.    Paratus was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 412043, citation number 1, item number 4b and was affirmed as a final order on 11/29/12, with respect to a workplace located at 45 Roberts Avenue, Buffalo, NY 14206.    NO ABATEMENT CERTIFICATION REQUIRED
