Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard associated with the collapse of the operator's platform.  On or about April 19, 2013, during workover operations  at the Turner 36-1 gas well the employer did not protect employees from the fall hazard associated with exceeding the 570 pounds weight limit load on the opereator's platform.  Four employee(s) with an estimated weight of 750 pounds were standing on the operator's platform attempting to recover a  downhole packer when the platform collapsed.
