(a) ABC Die Cutting and Embossing, 6363 E. 38th Ave., Denver, CO 80207; On and before April 30, 2013, the exposing employer failed to ensure that handrails were provided on the exposed sides of two stairways used to access the large format platform between the two large format embossing machines. Condition exposed employees to trip and fall hazards as they accessed the platform to utilize the large format machines.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables where not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. (a) ABC Die Cutting and Embossing, 6363 E. 38th Ave., Denver, CO 80207; On and before April 30, 3013, the exposing employer failed to ensure that strain relief was provided on two flexible cords utilized to provide power to the Jogger Aerator Machine, and to the area around the Federal Pacific Transformer Company Slitter machine. In both instances, power cords were dropped from strutural steel ceiling joists with no strain relief provided. Condition exposed employees to electric shock hazard as the power cords were used to provide electric power to machines on the floor and insulation was subject to damage.
