29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: (a) Regional Support Logistics Center Warehouse; On or about April 25, 2013, employees were working adjacent to four square holes measuring 23 inches wide X 23 inches high X 12 inches deep and one round hole measuring 24 inches in diameter and 12 inches deep while installing a block wall.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose: (a) Safety Trailer at Regional Support Logistics Center Construction Site; A white relocatable power tap was plugged into a black relocatable power tap powering office equipment including a stereo, computer and printer. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (b) Safety Trailer at Regional Support Logistics Center Construction Site; A toaster oven was plugged into a white relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances. This exposes employees to possible electrical fire hazards.
