29 CFR 1926.451(f)(6): The clearance between scaffolds and power lines were less than 10 feet:  a)  1509 Frankford Avenue - Employees were applying stucco from a tubular welded frame scaffold approximately 2 1/2 feet from a high power line of approximately 7.6kv to ground, exposing employees to an electrocution hazard.  Observed on or about April 23, 2013.

29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer:   a)  1509 Frankford Avenue - Tubular welded frame scaffold with a height to base ratio of more than four to one (4:1) was not restrained from tipping by guying, tying or bracing.  Employees were applying stucco on the 4th working level, approximately twenty-six (26) feet above ground level.  Observed on or about April 23, 2013.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a)  1509 Frankford Avenue - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform, exposing employees to a fall hazard of approximately twenty-six (26) feet.  Employees were using the side frames of the scaffold to access and/or egress from the working levels.  Observed on or about April 23, 2013.
