29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    a) On or about April 30, 2013, at the job site where a roller coaster was being erected, lifelines were used for access to the work areas. The employer did not inspect the lifelines to ensure they were of sufficient lengths for accessing the varying areas of work including the ground level. An employee suffered significant injury when descending from his work area to the ground level, in that, the lifeline to which he was attached was too short and he fell 18 ft.
