29 CFR 1926.404(b)(1): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. On or about April 30, 2013, the employer was using their portable ground fault circuit interrupters in-line with their extension cords and not connected at the main source of the power.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about April 30, 2013, the employer was using a large electric pig-tail box that was connected to the main power source. The electrical cord was pulled away from the box and there was no strain relief.
