KeHE Distributors, Inc.: The employer failed to ensure an eyewash shower unit was adequate and/or available in areas where employees were exposed to injurious materials such as, but not limited to, sodium hypochlorite, XL-50 (Potassium Hydroxide), lead/acid batteries and other injurious materials while performing maintenance, forklift operations, sanitation operations and other activities involving warehouse and food distribution.    a) Clean Room: An eyewash/shower facility was not provided for exposed employees.    b) Battery charging Area (near the dock): The eyewash/shower unit was inadequate, in that the eyewash bowl was not secured; the water pipe did not have a connection to prevent water flow on the floor.
