29 CFR 1910.1030(h)(5)(i)(A): The employer did not maintain a sharps injury log that contained at a minimum the type and brand of device involved in the incident: a) Alden-Lincoln Park Rehabilitation and Health Care Center, Inc., Chicago, IL- The employer did not maintain a sharps injury log. The privacy case entered on the 2013 OSHA Form 300 Log of Work-Related Injuries and Illnesses as entry #lp-3 for needle stick injury which occurred on May 24, 2013 was not recorded on a sharps injury log with the type and brand of device involved in the incident. b) Alden-Lincoln Park Rehabilitation and Health Care Center, Inc., Chicago, IL- The employer did not maintain a shaps injury log. The privacy case entered on 2013 OSHA Form 300 Log of Work-Related Injuries and Illnesses as entry# lp-1 for needle stick injury which occurred on February 22, 2013 was not recoded oon a sharps injury log with the type and brand of device involved in incident. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
