Site - On or about April 26, 2013, barricades were not utilized in areas where employees were exposed to falling materials.

Site - On or about April 26, 2013, fall protection was not utilized by employees while performing framing work on the roof area.     Instance b. First floor level - On or about April 26, 2013, door and window openings were not protected or guarded to prevent fall hazards.  Instance c. Second floor level - On or about April 26, 2013, window openings were not protected or guarded to prevent fall hazards.  Instance d. Third floor level - On or about April 26, 2013, window openings were not protected or guarded to prevent fall hazards.   On The Level Construction was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29CFR 1926.501(b)(10), which was contained in OSHA inspection number 312542319, Citation Number 1, Item Number 2, issued on 10/17/2008, with respect to a workplace located at . Baxter Boulevard Portland, ME. The final order date for the citation was on 11/21/2008.     On The Level Construction was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29CFR 1926.501(b)(15), which was contained in OSHA inspection number 312542319, Citation Number 1, Item Number 3, issued on 10/17/2008, with respect to a workplace located at .
