29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a.  On or about April 29, 2013 - at the above addressed jobsite, the employer did not initiate and maintain an accident prevention program and/or safety and health program to comply with the regulations of this part which included, but were not limited to employee protection when exposed to live electrical parts.

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  a.  On or about April 29, 2013 - at the above addressed jobsite, an inspection was not conducted by a competent person capable of recognizing and correcting hazards associated with electrocution, which could result in employee injuries.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a.  On or about April 29, 2013 - at the above addressed jobsite, employees were walking/working around, and/or using electrical outlets and light switches that were not provided with covers or face plates to protect against accidental contact, thereby exposing the employees to electrical hazards.

29 CFR 1926.405(d): Switchboards and panelboards. Switchboards that have any exposed live parts shall be located in permanently dry locations and accessible only to qualified persons. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons. Exposed blades of knife switches shall be dead when open.  a.  On or about April 29, 2012 - at the above addressed jobsite, four (4) electrical panels were not protected with covers, thereby exposing employees to arc flash/blast hazards.
