29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. On or about April 25, 2013, at the above addressed jobsite, an employee was exposed to a fall hazard of approximately 15 feet while using a portable extension ladder that did not extend 36 inches above the upper landing at 20880 Biscayne Blvd, Aventura, Fl. 33180.
