29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a. On or about April 25, 2013, at the above addressed job site, employees were exposed to a fall hazard while performing masonry work from a fabricated frame scaffold which was not equipped with access ladders. Genexis Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(c)(2), which was contained in OSHA inspection number 314843384, citation number 2, item number 1, and was affirmed as a final order on October 1, 2010, with respect to a workplace located at 3995 Van Dyke Dr., Lutz, FL 33067.
