29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    (a) In the Tarp Repair Area, 50 foot extension cords were used to supply power to heat guns, but there was not adequate inspection of the condition of the extension cords and the condition of their outer jacket and insulation.  For example, on May 8, 2013 an extension cord was being used which had breaks and tears on its outer jacket, with exposed inner wire(s) and bare copper visible.  This condition potentially exposed workers to a severe electrical shock hazard.
